Acupuncture and IVF
Acupuncture is a component of traditional Chinese medicine that
can be traced back for at least 4000 years. Acupuncture is based on
the theory that disruption of energy flow (Qi) along pathways known
as meridians can result in disease. Acupuncture involves the
placement of thin metallic needles in the skin along certain points
that connect these meridians. It has been used over the centuries
to treat various ailments and more recently has been used to treat
fertility problems. The use of acupuncture as an adjunct therapy in
the treatment of infertility has been the subject of several
studies over the past few years. The studies vary in their design,
the location of needle placement and the timing and frequency of
treatments. Most of the studies have been done in the context of an
IVF cycle. Although not all individual studies have demonstrated a
benefit, when viewed as a whole, the studies do suggest a modest
benefit on live birth rates with IVF. A recent review of the use of
acupuncture in IVF cycles (British Medical Journal, Aug 15,
2008) did indicate a statistically significant improvement in
pregnancy rates with acupuncture compared to placebo. The number
needed to treat to obtain one additional live birth with
acupuncture was nine. Stated otherwise, nine women would need to do
acupuncture with their IVF cycle to achieve one additional live
birth that would not have otherwise been achieved.
The mechanism by which acupuncture may improve pregnancy rates
is not known. It has been hypothesized that acupuncture may alter
certain hormones and neurotransmitters that affect the uterus and
ovaries. These hormonal changes may improve uterine blood flow and
relax the uterus at the time of embryo transfer, resulting in
improved embryo implantation. It should be noted that in the
studies that have shown an improvement in pregnancy rates,
acupuncture was performed twice on the day of embryo transfer; once
shortly before transfer and again shortly after embryo transfer.
We do not discourage the use of acupuncture in patients
undergoing IVF, although we believe the data on its benefit is
still inconclusive. If you elect to do acupuncture during your IVF
cycle we recommend you use a licensed acupuncturist and have the
procedure performed on the day of embryo transfer. You should
inform your doctor if you are doing acupuncture or are receiving
any other alternative medical therapy.
