Initial Patient Visit

Before Your First Visit

If you have had any previous diagnostic tests or treatments, you should request that your physician send copies of your records to:

This should include notes from any office visits with your physician, lab results, x-ray results and operative reports from any gynecology related surgery. A written authorization to release your records is usually required. If you have had prior surgery, you should contact the medical records department of the hospital where the surgery was performed and request a copy of the operative report be sent to our office. If your husband has had any fertility evaluation including a semen analysis, these records should also be requested. If you have any of these records in your possession, please bring them with you to your appointment.

After you schedule your appointment our office will send you a new patient information packet. In this packet you will find a form entitled “American Society for Reproductive Medicine, Infertility History Form”. This form has detailed questions concerning both female and male medical history. You and your partner should complete the appropriate parts of this form, sign it and bring it with you to your first visit. We strongly suggest you complete this at home prior to your appointment. This form is quite lengthy and your appointment will be delayed if you have to complete it in our office. If you are not seeing us for infertility, but for a gynecologic problem, you should still complete the applicable portions of this questionnaire.

You may also fill out your medical history online by clicking on the “Patient Portal” link on the home page of our website. In order to use this online form you will need a user ID and a password. These will be given to you when you make your initial appointment. If you have already made an appointment and did not receive these you may call our office and you will be given an ID and password.

We will contact your insurance company to verify your insurance benefits. Insurance companies in Arkansas vary in their coverage for infertility services. When you check in for your initial appointment we will let you know what insurance coverage you have.

The First Visit

If your visit is for infertility, we suggest that your husband come with you to this appointment. Infertility is a couple’s problem and we need the active participation of both the wife and the husband. The physician will review all of your previous records and take a comprehensive medical history from you. A physical exam, including a pelvic exam, is usually performed along with a transvaginal ultrasound exam of your uterus and ovaries. At the conclusion of this visit the physician will review the findings of your exam and ultrasound. It is not always possible to arrive at a diagnosis or formulate a treatment plan at this time. Additional testing is sometimes required. This may include hormone testing, x-rays or additional ultrasounds. Depending on your circumstances, we may ask that your husband produce a semen sample for analysis at your first visit.

The Next Step

You should schedule a follow-up appointment with your doctor after the additional testing has been completed. At this time he will review all of your results and a treatment plan will be finalized. It is very important that you return for this visit so that you can start your treatment.
